The women in the backseat is his love and the hammer she's hanging a hammer over his head which is metaphorically her actions in that she is stringing him along and dragging things out that are bringing issues to their relationship. I may be wrong but this song really resonated with me as I have found myself in a situation that parallels to these lyrics.
